Purpose To compare overall survival between patients who received neoadjuvant therapy (NAT) followed by resection and those who received upfront resection (UR)-as well as a subgroup of UR patients who also received adjuvant therapy-for early-stage resectable pancreatic adenocarcinoma. Patients and Methods Adult patients with resected, clinical stage I or II adenocarcinoma of the head of the pancreas were identified in the National Cancer Database from 2006 to 2012. Patients who underwent NAT followed by curative-intent resection were matched by propensity score with patients whose tumors were resected upfront. Overall survival was compared by using a Cox proportional hazards regression model. Early postoperative and oncologic outcomes were evaluated. Results We identified 15,237 patients with clinical stage I or II resected pancreatic head adenocarcinoma. From the NAT group, 2,005 patients (95%) were matched with 6,015 patients who underwent UR. The NAT group was associated with improved survival compared with UR (median survival, 26 months v 21 months, respectively; stratified log-rank P < .01; hazard ratio, 0.72; 95% CI, 0.68 to 0.78). Patients in the UR group had higher pathologic T stage (pT3 and T4: 86% v 73%; P < .01), higher positive lymph nodes (73% v 48%; P < .01), and higher positive resection margin (24% v 17%; P < .01). Compared with a subset of UR patients who received adjuvant therapy, NAT patients had a better survival (adjusted hazard ratio, 0.83; 95% CI, 0.73 to 0.89). Conclusion NAT followed by resection has a significant survival benefit compared with UR in early-stage, resected pancreatic head adenocarcinoma. These findings support the use of NAT, particularly as a patient selection tool, in the management of resectable pancreatic adenocarcinoma.

Results and limitations:  We identified a total of 4639 eligible patients with mRCC. Among the 4202 patients treated with targeted therapy and 437 patients treated with immune checkpoint inhibitors, 2326 (55%) and 234 (54%) patients received upfront CN prior to treatment start. In multivariable analyses, CN was associated with significantly better OS in both the immune checkpoint inhibitor-treated (hazard ratio [HR]: 0.61; 95% confidence interval [CI], 0.41-0.90, p = 0.013) and the targeted therapy treatment (HR: 0.72; 95% CI, 0.67-0.78, p < 0.001) group. There was no difference in OS benefit of CN between the immune checkpoint inhibitor and targeted therapy treatment groups (interaction p = 0.6). Limitations include selection of patients from large academic centers and the retrospective nature of the study.

I always require full payment upfront for video projects. There's one client that I've conducted 3 phone calls (usually it's one call max- I prefer email) and the project is only $350. He refuses to pay upfront and says he will do 1/2 upfront and 1/2 after. He runs a very successful business though and I was hoping to collaborate on more projects. How would you handle this?

Background:  Studies comparing upfront surgery with neoadjuvant treatment in pancreatic cancer may report only patients who underwent resection and so survival will be skewed. The aim of this study was to report survival by intention to treat in a comparison of upfront surgery versus neoadjuvant treatment in resectable or borderline resectable pancreatic cancer.

Results:  In total, 38 studies were included with 3484 patients, of whom 1738 (499 per cent) had neoadjuvant treatment. The weighted median overall survival by intention to treat was 188 months for neoadjuvant treatment and 148 months for upfront surgery; the difference was larger among patients whose tumours were resected (261 versus 150 months respectively). The overall resection rate was lower with neoadjuvant treatment than with upfront surgery (660 versus 813 per cent; P < 0001), but the R0 rate was higher (868 (95 per cent c.i. 846 to 887) versus 669 (642 to 696) per cent; P < 0001). Reported by intention to treat, the R0 rates were 580 and 549 per cent respectively (P = 0088). The pathological lymph node rate was 438 per cent after neoadjuvant therapy and 648 per cent in the upfront surgery group (P < 0001). Toxicity of at least grade III was reported in up to 64 per cent of the patients.

I just had an unpleasent experience with this new payment feature. Guest reserved a few months in advance for a very sought after perior or time in Fort Lauderdale. He then used the Pay less upfront option. A few days later he cancelled the resrrvation. We have a strict cancellation policy, which should have allowed me to receive 50% of the etire booking, inspead I received 50% of the 50% he already had paid. I feel disapointed and cheated. Me: I just got a call from a guest who rented my place starting 9/1. She wanted to know if I am taking the second payment today. I don't handle payments, you guys do so I directed her to contact you. After getting this call, I found out ABB has some policy called "pay less upfront". How do I opt out of this? I don't want the risk of someone staying without paying or cancelling and expecting a refund and I'm left holding the bag.

ABB: I would like to share with you that the in payless upfront option guest can pay in 2 payments.

First payment is taken at the time of booking and the second after few days, however by any chance the guest fails to pay the second payment the booking gets canceled automatically within a week.

So, even in that case hosts never have to deal with a guest who has paid in half. When a guest checkin they have already paid the full amount.

I asked two different case managers where I could find the information/notification on my account that a guest is using the "pay less upfront" option, and why Airbnb are leading us to believe that we have fully paid up, confirmed bookings when they're nothing of the sort. One told me that a guests' financial dealings are none of the host's business, and the other told me that as a responsible host, it's my duty to respect my guests' privacy! (From anecdotal evidence, it does appear that some multiple-listing commercial property managers are privy to this information, but in most jurisdictions at least, traditional hosts are still not being informed that a guest is utilising the 50% now/50% later facility)

I searched for info on this but could only find an article (not on Airbnb) stating that guests who "pay less upfront" can cancel up to 30 days out and get a full refund, this would nullify the 'strict/grace' policy: full refund only if the guest cancels within 48 hours of booking.

Boucly and colleagues should be commended on this analysis with relevant implications for patients with newly diagnosed PAH. The study is the largest real-world cohort to support the overall treatment strategy from the 2018 World Symposium (2). It adds to the observational evidence for upfront combination therapy including a parenteral prostacyclin for high-risk patients. More precise risk stratification, potentially including parameters of right ventricular function, might inform more personalized treatment decisions.
