Step 1:
Compile all required documents and make sure you have clear photocopies of each. You should also be able to secure your two (2) pieces of 1x1 and two (2) pieces of 2x2 ID photos in advance. Finally, organize all these items inside a long brown envelope and a plastic envelope, along with two (2) white folders, to keep your records protected.
Step 2:
Proceed to the Nursing Department’s Office to continue with your pre-enrollment process. Request your copy of the Personal Data Sheet (PDS) and the Enrollment Progress Tracker (EPT). Then, secure your Entrance Exam Referral Slip to proceed with your entrance examination.
Step 3:
Pay the entrance exam fee of ₱300.00. Present the receipt along with the referral slip at the Guidance Center.
Note: Exam results must be claimed within 2–3 days after taking the exam.
Step 4:
Once you get your entrance exam results, take a photo of them to have your own copy. Proceed back to the Nursing Department’s Office for submission, where you will then be interviewed.
Step 5:
After the interview, you will receive a Medical Referral Slip to proceed with your medical examination at UC-Med or SVGH.
Step 6:
Present your medical results at the UCLM Medical Clinic, located on the ground floor of the Old Maritime Building, across from the University Chapel.
Step 7:
Submit the original medical certificate issued by the physician to the Nursing Department’s Office. Do not forget to retain a copy for yourself.
Step 8:
Create your student account at https://lmportal.uc.edu.ph.
Step 9:
Once your portal account is secured, you can return to the Nursing Department's Office to receive your UC FORM 1-A.
Note: Secure your original documents and submit them as you complete the last step of your enrollment at the RECORDS SECTION/REGISTRAR'S OFFICE.
The interview is done face-to-face.
