The Transition Program serves students ages 18 to 21 who have completed graduation credit courses but whose IEP team has determined the need for additional support in a Community Transition Program prior to graduating. The Program offers a continuance of services outlined in each student’s Individualized Education Program (IEP) and Person-Centered Plan (PCP).
Services may include some or all of the following:
Curriculum and Assessment; Community Work Based Learning; Update to PCP (annually); Student led IEP (annually); Living Skills, State ID, SSI and DDA (if necessary); DVR; Post-Secondary Education and/or Training; Career Related Learning Experiences; Community Based Instruction; Transportation Training; and Student Portfolios.
