Complete steps 1 - 6 below. It will be helpful to read the steps before you begin.
2. W-9 IRS Form - Participants MUST have a valid Social Security number or Tax ID to complete this form and to get your stipend.
4. Complete your online trainings and upload your certificate of completion

Participants who complete Transfer Bridge will receive a refund of their deposit.
Bridgers can review what our office has received below. Use your last initial and the birth 'day' of your birthday to find your student code. For example If Joe Bruin was born on January 1, 2000, then his student code will be B01.
All materials MUST be submitted and received by our office by the start of the program.

Our Welcome and Orientation is scheduled for Tuesday, September 3 from 1 - 6PM in the Nancy & Aaron Cohen Room located on the 1st floor of Engineering VI. The building is located at 404 Westwood Plaza, Los Angeles, CA 90095, at the top of Westwood Boulevard, and directly across the Luskin Conference Center. Please arrive on-time and no later than 1PM.
De Neve - Dogwood Residential Building - is near the De Neve Plaza located at 351 Charles E. Young Drive West, Los Angeles, CA 90024.
Rooming is triple-occupancy. You will be provided a linen-pack (that includes, a blanket, pillow, sheets, and a towel) from the start of the program until the 9/15 move-out. Residents will be expected to provide their own accommodations after that date. Weekly cleaning services of the common areas will be provided. There is complimentary wifi in the residential areas. Finally, you will have access to the recreation facilities during open hours. Recreation hours will be updated weekly and can be found here: https://www.recreation.ucla.edu/facilityhours.
You are responsible for returning your key upon check-out and will be charged if it is not returned.

Participants are registered for the American Meal Plan. This provides 3 meals per day starting with dinner on arrival day and lunch on the departure day.
Sack lunches - Each participant will be provided a complimentary insulated bag that can be used to request a sack lunch for your days at Bridge. We recommend participants to utilize this option as the walk from our program site to the dining halls may not be enough for you to enjoy your meal.
The sack lunch can be requested Monday through Friday (until 9/13) at Rieber Hall Dining only. You must bring your insulated bag to request a sack lunch.

Move-in Parking - Participants can purchase a parking permit ($13) for DD Structure (nearest to Dogwood) at the De Neve Front Desk. Additional parking can be found at Sunset Village Parking Structure or Parking Structure 11 and a limited amount of pay-by-the-hour spots are available in this lot. Please note that SV and Structure 11 are located further away from our housing facility.
