1. Read, complete, and scan all your administrative forms
   • Transfer Bridge Contract
   • Activities Waiver
   • Elective Voluntary Activities Waiver
   • Evidence of Health Insurance
   • Copy of Cards (ID & Insurance Card)
   • Publicity Release

2. W-9 IRS Form - Participants MUST have a valid Social Security number or Tax ID to complete this form and to get your stipend.

3. Click here to complete your Transfer Bridge online registration.

4. Complete your online trainings and upload your certificate of completion

• EH & S Lab Safety Fundamental Concepts (LSFC)*, and
• EH & S Shops training (IP-SHOP)*

*Access the trainings at UCLA Worksafe website. You will need your UCLA Log On ID to access the system. Links are provided on the Worksafe site.

5. Click this link to access the uploader to submit necessary materials. Please save your files as follows:

• Administrative Forms and Contract - complete all forms and save as one .pdf file by LastnameFirstname_AdminForms.pdf
• Unofficial Transcripts - save all transcripts as one .pdf file with the file name: LastnameFirstname_Tran.pdf
• Training Certificates - save both certificates as one .pdf file with file name: LastnameFirstname_Cert.pdf
• Facepic of at least 300 dpi. File name: LastnameFirstname_Pic.jpg

6. Mail your completed W-9 Form and $100 personal check deposit to secure your spot to:

Our Welcome and Orientation is scheduled for Tuesday, September 3 from 1 - 6PM in the Nancy & Aaron Cohen Room located on the 1st floor of Engineering VI. The building is located at 404 Westwood Plaza, Los Angeles, CA 90095, at the top of Westwood Boulevard, and directly across the Luskin Conference Center. Here is a link to the building on Google Maps.

Please arrive on-time and no later than 1PM.

• T-shirt from your community college (if you have one) for our group photo
• Calculator (if you have one)
• Open mind, great attitude, and readiness to work

Housing Dates

• August 29, 2019 (9 - 11 AM) | Transfer Bridge Mentor move-in (mentors only)
• September 3, 2019 (9 - 11 AM) | All Transfer Bridge participants move-in
• September 15, 2019 (by 11 AM) | Move-out
  • Participants & mentors who DO NOT have fall UCLA Housing will need to move-out and commute for the duration of the program.
  • Transfer Bridge participants & mentors who have fall UCLA Housing will move into their fall room assignment (information on this process will be provided prior to the weekend move). Transfer Bridge will be billed for the additional nights.

Our Housing Facility

De Neve - Dogwood Residential Building - is near the De Neve Plaza located at 351 Charles E. Young Drive West, Los Angeles, CA 90024.

Rooming is triple-occupancy. You will be provided a linen-pack (that includes, a blanket, pillow, sheets, and a towel) from the start of the program until the 9/15 move-out. Residents will be expected to provide their own accommodations after that date. Weekly cleaning services of the common areas will be provided. There is complimentary wifi in the residential areas. Finally, you will have access to the recreation facilities during open hours. Recreation hours will be updated weekly and can be found here: https://www.recreation.ucla.edu/facilityhours.

You are responsible for returning your key upon check-out and will be charged if it is not returned.

Participants are registered for the American Meal Plan. This provides 3 meals per day starting with dinner on arrival day and lunch on the departure day.

Sack lunches - Each participant will be provided a complimentary insulated bag that can be used to request a sack lunch for your days at Bridge. We recommend participants to utilize this option as the walk from our program site to the dining halls may not be enough for you to enjoy your meal.

The sack lunch can be requested Monday through Friday (until 9/13) at Rieber Hall Dining only. You must bring your insulated bag to request a sack lunch.

Here are our dining facilities (based on dates):

• Rieber & De Neve Dining is open to us from 9/3 - 9/14.
• Rieber is open on 9/15.
• Sproul Dining is open from breakfast on 9/16 - 9/21 (for those with fall UCLA housing contracts).

• Lunch | 11 AM - 2 PM
• Dinner | 5 - 8 PM
• Participants can check Rendezvous dining for hours and availability.
• Residents can also look to campus eateries or restaurants in nearby Westwood for other options. No funding for meals are provided beyond the meal-plans included in the Transfer Bridge award.

Move-in Parking - Participants can purchase a parking permit ($13) for DD Structure (nearest to Dogwood) at the De Neve Front Desk. Additional parking can be found at Sunset Village Parking Structure or Parking Structure 11 and a limited amount of pay-by-the-hour spots are available in this lot. Please note that SV and Structure 11 are located further away from our housing facility.
