Entry Supervisor

• Knows the hazards, symptoms and consequences

• Verifies the permit by determining acceptable entry conditions exist

• Authorizes entry

• Oversees entry operations

• Terminates entry

• Verifies rescue services

• Removes unauthorized individuals

• May serve as attendant

Attendant

• Knows the hazards, symptoms and consequences

• Aware of potential behavioral effects

• Monitors entrants and maintains count

• Monitors hazards and activities inside and outside permit space

• Remains outside entry point

• Communicates with entrant

• Controls entry point
