• 1. Permit space to be entered

  2. Purpose of the entry

  3. Authorized entrants and attendants

  4. Entry supervisor and place for signature

  5. Hazards of the permit space

  6. Isolation measures

  7. Acceptable entry conditions

  8. Test results with the name of tester and time

  9. Rescue/emergency services available and means to summon

  10. Communication pocdures between entrant and attendant

  11. All necessary equipment and information
