While I respect Oda's decision I have it Headcanoned that Izou survives the battle of Onigashima. While severely wounded from his encounter with CP-0, he is found in time by Marco to be brought back from the brink of death with the healing flames of Marco's power. After recovering from his wounds he remains in Wano, reclaiming his former position as a retainer of the Kauzuki Clan. Due to his knowledge of the outside world, he becomes a key advisor to Momonosuke.

When Izou left Wano with Oden he found that the sake of the world outside of their borders was lacking the flavors and long perfected art of his people. Due to this, he began learning how to make sake on his own. Tirelessly working to perfect his recipes and techniques until he mastered the art. Creating different flavor palettes for the various lands that they visited, including his specialty. Sakura Sake, his own personal favorite blend is a reminder of his home. 

Since he was a child, Izou learned how to perform Shibu, the art of fan dancing. This was done at first to earn him and his sister money on the streets of Wano. After they joined Oden he continued with the art out of true enjoyment for the art and the fact that he was extremely skilled at it. Soon becoming more well-known than even the best geisha of the time. It was due to his hours of practice and use of the art as meditation that Hiyori learned how to dance with the fans when she was younger. A skill that unknowingly would help her in the future when she took on the disguise of Komurasaki.
