STEP TWO - Do this:
Start with an informal interview similar to this one CLICK FOR LINK and have the student complete the Career Cluster Inventory on MissouriConnections [info page] OR see my Teacher Resources page - look at the Transition Assessments section for alternative Transition Assessments (there are several printable options if technology is limited or unavailable).
STEP THREE - The IEP:
Now use the information you have collected in the IEP's Present Level Transition section and Transition Plan.
