You MUST complete one form per registrant.  You will have the option to submit a new registration upon completion of this form.

AGE REQUIREMENTS

Pre-K 3 participants are welcome, however must have completed one year of a Pre-K program and must be potty trained (no diapers/pull-ups)

By registering for Scuba VBS at Terryville Congregational Church you consent to your child's photo and/or video being taken for marketing purposes while on church premises.  Terryville Congregational Church has permission to share your social networking photos to our social media pages for promotional purposes.  Please note names will not be published.  

We are collecting email addresses for future communication.   By registering for TCC VBS you consent to receiving regular email updates from us for VBS purposes.
