   Complete the last page and sign it. If Married Filing Jointly, both spouses must sign. This is a fillable form.

2. AARP Intake questionnaire (Required)
   Complete the questionnaire and sign it. If Married Filing Jointly, both spouses must sign. This is a fillable form.

3. Itemized Deduction Worksheet (Required if planning to itemize)

4. What to bring or check list
   Make sure to bring all required personal identification, social security cards and relevant tax documents.
