Fill in the forms before you arrive for your appointment

If you can't download, you can pick up a packet at the Gum Spring Library.

However, paper packets are limited so download the forms if you can.

1. Taxpayer Checklist (Make sure to bring all required personal identifications, social security cards and relevant tax documents.)

2. IRS Form 14446 (Required) Complete the last page and sign it. This is a fillable form. If married, both taxpayers must sign the form.

3. AARP Intake Questionnaire (Required) Complete the questionnaire and sign it. This is a fillable form.

4. Itemized Deduction Worksheet (Required, If you are able to itemize)

5. Education Credits Worksheet (Required, if you have education expenses for higher education)

6. Self-Employment Worksheet (Required, if you have self-employment income, 1099 Misc, 1099 NEC)
