Interpretive Summary: Demand for longer, stronger, and more uniform fibers for the production of higher-quality yarns has led to an increased emphasis on the genetic improvement of cotton fiber quality. To date, cotton breeders have predominantly relied on conventional breeding methods for the improvement of fiber quality by making crosses between elite cultivars and selecting progeny based on measurable fiber properties. However, DNA marker-assisted selection has the potential to increase the efficiency of selection, particularly concerning the simultaneous improvement of fiber quality and lint yield. Over the past few decades, hundreds of DNA markers linked to fiber quality traits have been identified, yet their utility in an applied cotton breeding program has not been well documented. In this study, we compared the efficiency of selection based on previously reported DNA markers for fiber quality traits to selection based on direct measures of fiber properties. We found that selection for both fiber length and strength based on six DNA markers was comparable to direct selection for plants in the top 20%. Although DNA markers are not likely to substitute for the physical measurement of fiber properties, our results suggest that DNA marker-assisted selection could improve the efficiency of selection for cotton fiber quality traits.

Technical Abstract: Marker-assisted selection (MAS) may help mitigate some of the current challenges regarding the genetic improvement of cotton fiber quality, such as low genetic diversity and the negative association between fiber quality and lint yield. A multitude of quantitative trait loci (QTL) for fiber length and bundle strength have been identified in the literature, but the use of MAS for the improvement of fiber quality is still rare in public cotton breeding programs. Validation studies are necessary to develop portable genetic markers and to identify QTL with stable effects on trait expression across environments and genetic backgrounds. The effects of previously reported microsatellite markers (SSRs) linked to QTL for fiber length and bundle strength were evaluated in three genetic backgrounds, and the efficiency of MAS for fiber quality utilizing SSRs linked to robust QTL was investigated. Using the results of 32 published QTL mapping studies, six SSRs linked to robust QTL for upper-half mean fiber length (UHML) and six SSRs linked to robust QTL for fiber bundle strength (Str) were identified. In all but one case, the genetic gain achieved through marker-based selection of F3 plants having four-to-six alleles in the desirable state for UHML and Str was similar to that achieved by phenotypic selection of the top 20%.

Very low quality evidence from controlled before-after studies (downgraded due to the risk of bias, inconsistencies and potential indirectness of outcomes) suggests that the implementation of care bundles may be an effective strategy to improve patient outcomes when compared with usual care. Future research should focus on the explicit and transparent reporting of the implementation of the care bundle including issues relating to implementation fidelity such as the frequency with which the behaviour change techniques were used. Evidence for the central line-associated bloodstream infection (CLABSI) bundle effectiveness remains mixed, possibly reflecting implementation challenges and persistent ambiguities in how CLABSIs are counted and bundle adherence measured. In the context of a tertiary pediatric hospital that had reduced CLABSI by 30% as part of an international safety program, we aimed to examine unit-based socio-cultural factors influencing bundle practices and measurement, and how they come to be recognized and attended to by safety leaders over time in an organization-wide bundle implementation effort.

At our study site, safety leaders made efforts to respond to socio-cultural factors influencing bundle implementation, which involved shifting between centralized and unit-based approaches. Our findings confirmed the importance of hospital prioritization of CLABSI prevention, a noted implementation facilitator [48]. This organizational-level priority gave CLABSI prevention work legitimacy at the clinical level, and allowed individuals within the unit to leverage this organizational mandate to support unit efforts. However, when CLABSI was not initially recognized as a problem worth targeting, the organizational priority setting was less impactful, and changes took longer to achieve impact; underlining the need for senior leadership to do more than identify a safety problem, they need to have bottom-up engagement in shared construction of the problem and solutions [49]. This insight was only recognized and acted upon over time. When pursuing a bottom-up approach (i.e., unit-based education and auditing), local resource constraints and unit-specific approaches led to variation in implementation. These challenges (and the perception that auditing did not help to reduce CLABSIs) prompted adopting a centralized approach to education and auditing along with regular leader CLABSI-focused rounding. Still, some socio-cultural factors such as professional interactions were more challenging to address and were not visibly done so at either level. Tensions between local and centralized approaches, as in the case of central auditors accessing units, continued to exist. CLABSI prevention targets were achieved despite less attention to these issues. However, this achievement does not detract from their likely importance to sustainable and widespread improvement [7], and the importance of attending to what is, and is not, captured through measurements in QI programs. Our findings reflect the need for continual attention to balancing and maintaining top level commitment in CLABSI prevention and local ownership in QI initiatives, as well as to complex patterns of professional interactions, unit norms, and hierarchical relationships [17, 50, 51]. 17dc91bb1f
