FIRST COMMUNION - Baptism Certificate, Medical Insurance Card

CONFIRMATION (Youth & Adult) - Baptism Certificate, First Communion Certificate, Medical Insurance Card

RCIA (Children, Teens, & Adults) - Medical Insurance Card. Varies per sacrament needed; if an individual has not been baptized, Birth Certificate is needed. If Baptized, even if it is not Roman Catholic, Baptism certificated is needed.
