Client Forms

All new clients must fill out the Client Information and Terms & Conditions forms.

Please read the preparation and maintenance guidelines in the info, before and aftercare sections for lash and eyebrow services.

Please note that additional waivers are required for these services.

If forms are not completed, time will be deducted from your in salon service to complete them; if it takes you more than 10- 15 minutes to complete them, your according to your service will be canceled/ completed at best in the timeframe remaining.
