FORMS and PAPERWORK

IMPORTANT NOTES

• For D1 teams, coaches can help with the forms. D2 and D3 teams should fill out their forms on their own.

REQUIRED FORMS

To find the forms:

1. Go to http://www.miodyssey.com/coaches-forms.html.

2. You will need the following forms:

THESE TWO FORMS ARE TURNED IN AT REGISTRATION ON COMPETITION DAY.

• • 1. Media Release Forms - 1 for each team member AND each coach. 

    2. Coaches Code of Conduct - 1 for each coach

THESE FORMS WILL BE PUT INTO THE ENVELOPE YOU RECEIVE AT REGISTRATION AND HANDED IN TO THE STAGING JUDGE AT YOUR COMPETITION ROOM.

    4. Cost - Click here to see an example of a Cost Form. Click here to see the Cost Assignment information in the Program Guide. Only 1 copy is needed for the team. 

    5. Style - Click here to see an example of a Style Form. You will need 4 copies. 

    6. Outside Assistance - only 1 copy is needed for the team.

       • NOTE: You should be able to put “None” on this form. If not, list the Outside Assistance elements but be aware that you will be penalized 1 - 25 points.

    7. Team Clarification - Team Clarifications are any clarifications submitted by your team. These do NOT include the General Clarifications from the OM website. Make sure you also include a copy of any TEAM Clarifications that pertain to your team (if any). You will need 4 copies of each team clarification.
