BPP16 Form - Rev. 8/04
All demolition applications must be pre-filed prior to requesting a pre-demolition inspection from the Construction Safety Compliance Unit (CSC). CSC will only accept applications for Inspection Prior to Demolition (CSC2A) with the nine-digit BIS job number on the upper right-hand corner of the form.
CSC2A Form - Rev. 1/19
Replaces CD10 form for CN applications only. However, CD10 form still applies for amending CD2 and CD3 forms. To be used to submit any change made after the original application has been approved.
CD4-A Form
This form is filed in addition to a CD8 form when special inspections are required to be performed on structural elements that support the crane or derrick. It replaces the use of a TR1 form, which will no longer be accepted by Cranes and Derricks for new applications filed after March 1, 2019. The crane design applicant who filed the crane or derrick notice application for the job determines when and which special inspections are required in accordance with 1 RCNY 3319-01, NYC BC Chapter 17 and 1 RCNY 101-06. Special inspections shall only be performed by registered special inspection agencies, approved for the project by the crane design applicant. The crane design applicant may perform these special inspections only if the crane design applicant is a registered special inspection agency.

Mast climber owners must only submit the MC2 form in conjunction with also submitting the MC-3 form for an approved CN application, or it will not be accepted. All other past uses of this form which were specific only to the device must now be done in DOB NOW: Build. The MC2 should be submitted with supporting documentation, which must include a list of the mast's critical components, the owner's certification and a disclosure of the mast climber's history.
MC2 Form - NEW Application
The POC3 form is to be used when an easement or restrictive declaration is filed in connection with a professionally certified application. The professional must sign the POC3 form, which will contain either the professional's certification: 1) that the easement or declaration materially conforms to the Department's approved form, or 2) that the easement or declaration has been reviewed and accepted by the General Counsel's Office.
POC3 Form - Rev. 12/08
Use this form to certify (a) the elevator application or building notice is complete and in compliance with all applicable provisions of law or rules; and (b) whether or not the application is being filed to legalize an existing condition.

The PD1 form allows the Department to collect plot diagram information in a uniform way for all applications. When a plot diagram is required, it is must be completed and submitted at time of Pre-Filing.

PD1 Instructions
The ZD1 is an 11"x17" title block for drawing that graphically summarizes the proposed zoning bulk, yards and street plantings. The ZD1 includes diagrams for site plans and other projections (3D or Axonometric as required) describing vertical dimensions. It will be required prior to approval with all initial filings for New Building and Alteration Enlargement applications. All applicable Post Approval Amendments (PAAs) will also require the ZD1.

This form will be used to capture the reasons why an application could not be processed prior to approval. This new form will accommodate B-SCAN so the applicant will be able to view the disapproval reasons online.
AO1 Form - Rev. 12/14
This form is required for all alternations or demolitions of buildings identified as single room occupancy multiple dwellings except for Alteration 3 applications for sheds, fences or curb cuts. This form assists the applicant in determining whether a certification of no harassment from the Department of Housing Preservation and Development is required to be submitted.
