On these occasions, the parent or guardian assumes full responsibility for following the Sample collection instructions and ensuring that the information that he or she provides to Cardiogram about his or her child is kept secure, and that the information submitted is accurate.

In the event we notify or become aware that the Service has been used by a child under the age of 18 to store information of that child without parental consent, we should be authorized to delete, in its entirety, with no notice to you, any of the information stored by that child or by you on that child’s behalf.
