There are two options to sign up for My FMC Portal. At your next scheduled visit you can provide your email address to a Fairfield Medical Center team member. They will be able to send you a secure email invitation to sign up for My FMC Portal. Through this email invitation you are able to create an account. This invitation to join expires after 90 days.If you do not have a scheduled appointment and wish to sign up for MY FMC Portal, you can contact the Medical Records Department by calling 740-687-8053, Option 4.The hours of operation are Monday- Friday, 7 a.m.-8:30 p.m. The staff members in the Medical Records department will be able to walk you through the process to sign up to the portal.

Patients 12 and under must have a proxy to access their records via My FMC Portal. Authorized Proxy access will be automatically revoked when the patient turns 13 years old, until 17 years old. Patients 13-17 may have their own portal account.
