Printout Part A & B of Informed Consent, Release Agreement, and Authorization - Part A & B (par C not required except for high adventure) -
MUST BE PAPER PRINTOUT, NOT ELECTRONIC
Required for each attendant, including cubs, siblings, parents, etc.
Why?
These are important for emergencies, and we want to have a physical printout on hand so that we can reference it and hand it off to a first responder or take it into the emergency room with us if needed.
The critical areas are especially:
Allergies
Medical allergies
Existing conditions
Emergency contacts
These are things that could save a life.
Which Parts?
Usually just Parts A & B. Part C is only necessary for high adventure activities.
Recommendation
We recommend this for making it easier in the future:
Download and fill out the PDF form on a laptop/desktop.
One form per person.
Save the PDF to your laptop/desktop, that way you can easily update it in the future or print out another copy.
Print a copy of each family member's form, fold them and place them in a gallon sized Ziplock for easy and protected carrying.
