Open Program->Membership-> Individuals Records from the Main Menu.
The Individual Information form contains basic information for an individual in a household or organization.
You may also open the Individual Records module from the Address Records window. Click Individual on the left sidebar will open Individual Records module, and list all records associated with that address.
Respect Individual preferences.
Follow the CDM+ guide for Individuals. The information below is supplemental.
Last Name
First Name
Preferred Name (often a nickname)
Title: rarely used
Suffix: rarely used
Family Status
Multiple “Heads of Household” are common.
Only one Family Status Code can be selected -- Sister cannot be a Head
Family Status codes are OSHPC-defined fields. These codes may be changed.
From main menu, select : Files -> Master Coding System -> Individual -> Family Status
Individual Address
Rare: use only if the individual’s address is different from the household (such as a military address), otherwise uses Household Address.
At the top of the Individual Form window are tabs to access additional individual’s fields.
Select a tab for additional fields.
Fields are available for several different telephone and email types.
Recording an email address is as important as a street address. Wherever possible, they should be obtained.
In today's environment, it is rare that an individual does not have some form of email address.
Recording personal data such as gender identity, age, education, and financial information requires approaching the process with the utmost sensitivity and care.
It is vital to ensure confidentiality, transparency, and strict compliance with data protection laws to maintain trust within the church community.
Members should be informed of how their information will be used, and consent should be obtained for its collection.
Mishandling such data can lead to privacy concerns and erode the sense of safety and inclusivity that OSHPC strives to uphold.
Since I know of no policy of OSHPC for the recording of personal information, except that required by the Book of Order, I do not recommend the recording of other personel information absent a clear policy guidance.
In the context of this tab, “membership” means the “Communicant Membership” of OSHPC.
The Clerk of Session is responsible (among other duties) for keeping an accurate and complete roll of the Communicant and non-Communicant membership of OSHPC.
The Clerk is not utilizing this function of the CDM+.
While the CDM+ makes provision for including photographs, this function is not systematically utilized.
Except for "Groups", OSHPC is currently not utilizing the remaining Tab functions
OSHPC Group codes enable characterization for virtually any type of individual-associated group information
E.g. Church Group membership, Church Offices (past and present), Time and Talent, interests and skills, etc.
Selecting the 🔽 icon at the end of the field displays field values defined by OSHPC. Only the eMail News group code is currently maintained regularly.
Recording an email address is as important as a street address. Wherever possible, they should be obtained.
The eMail code determines the distribution publication of “This Week at Old South Haven Church” and most other bulk eNotice distributions.
If an individual is to receive “This Week …” and other OSHPC-wide mailings, it is essential that an eMail address be recorded and the Group Code "eMail News" be assigned.
