• Due to a Sarkic related incident 'Onslaught' suffered a burst arteries on the left side of his neck. Surgery was preformed to reattached the arteries and stop the bleeding. No long term affects from this incident.

• While investigating a Sarkic drawing and plaque in solitary confinement a knife was shattered, lodging itself in the collar bone/shoulder region. It seems 'Onslaught' himself went to Medical Bay and pulled out the knife shard, wrapping the wound itself, thus leading to excessive bleeding. Later on 'Onslaught' would be taken to Medical Bay and undergo surgery due to getting hit by a Sarkic acolyte, causing a burst artery on the right side of his neck. During this surgery the artery was reattached and then proper medical attentional was administered on the previously mentioned shoulder/collar bone wound. Scarring is expected around the collar bone/shoulder region.

• During a set of assassination attempts at personnel on the site by a GOI known as the "Driftford Militia" 'Onslaught' was hit by a IED from a distance of no more then five feet, occurring just outside of warehouse on surface. This attempt coincided with the other attempts, the time varying at most 30 seconds. Due to the distance and the other assassination attempts medical treatment for 'Onslaught' was delayed with an estimation of 5-10 minutes, ultimately resulting in sever blood loss. During medical treatment it was decided that the left hand could not be saved, with the rest of the arm being badly broken and lacerated but still being able to be saved. His leg was also badly broken and lacerated but luckily could be kept. Overall most of the left side of the body suffered sever lacerations and breaks. Shrapnel was removed and lacerations stitched up, furthermore both the leg and the arm had the breaks set. SCP-2295 "Patchworks" was brought in to assist with the hand. Once the hand was cut off SCP-2295 "Patchworks" created a new hand with a colour matching 'Onslaught's skin tone. After surgery 'Onslaught' was instructed to take pain medication and to stay passive while the breaks healed and stitches could come out. Long terms affects after this incident include a patchwork hand and sever scarring along the left side of the body. 

• On 11/4/2024 during a major sarkic incursion involving several Acolytes and the Archon 'Tartarus' suffered eight broken ribs and a broken arm on his left side. Tissue damage was treated and stitched up, additionally the broken bones had plates put in to to help make sure they heal correctly.

• On 12/30/2024 XI-8 were deployed to Port Angeles to handle and stop a Adytum's Gate assault. During this operation 'Tartarus' received several sever lacerations, spanning across his body that required immediate medical attention upon returning to Site-66. Medical personnel fixed the tissue damage and stictched the wounds 'Tatarus' had encured closed. No lasting damage or affects are expected to occur from the injuries. 

Psychological Report

Onslaught is known to be a kind person to people he knows. However people that he finds to oppose him will receive none of that kindness. It is also of note that Onslaught will do what it takes to accomplish his goal as long as the ends justify the means. This is a mentality that he tends to use in all relevant circumstances within the confines of Site Guidelines and Objectives.

'Tartarus' seems to show no signs of remorse or negative emotion at killing those deemed to be in opposition to the Foundation and his tasks/objectives as XI-8. Among those he considers allies and friends 'Tartarus' is quite friendly and positive in general. 'Tartarus' seems to utilize a metaphorical "On/Off" switch in his attitude and mentality, separating his duties as XI-8 and personal matters when not preforming duties.
