Services
Services provided, are designed as a comprehensive package of regularly scheduled, individualized, health-related and assisted services which include: 24 hour on-site supervision and assistance, nutritional congregate dining, community integration, central storage of medications, and individualized assisted services provided to tenants in their own apartments. Individualized service plans will assess for skill deficits and address maintenance of community tenure through: integrated illness and behavioral management, assistance with activities of daily living and independent living skills, psychosocial skill development with activities of daily living and behavioral management. All service delivery is based on evidence-based best practices.
Assisted Services are divided into three general categories: integrated mental health/substance abuse, "homekeeping", and community building. It is essential that the specific services provided within these categories be, first and foremost, tenant driven and flexible enough to address the needs and interests of each individual. People need to feel safe and comfortable in their own homes. Services provided are all based on the concept of empowerment and respect.
Services are designed to continually encourage independent living skills to improve tenants' baseline level of functioning, to minimize breakthrough symptoms and decompensation, to provide crisis prevention services in a manner that allows tenants to remain in their homes if at all possible, and finally, to improve overall quality of life.
