Please do this even if you previously checked in on the app or wrote you name down on paper

Step 1

Fill out this Google form for EVERY session that you attended.

• The session codes can be found on this sheet. 

Step 2

You are required to fill out this CTLE Program Evaluation Form for every session you attended. 

Step 3

Once your evaluations are completed, the final step is to  fill out and submit this CTLE Request Transcript Form.

Please submit eerything by June 6th.
