It's with great pleasure that Avai Football Club receives the athlete Nickolas LE 04 for a period of evaluation at the club. The athlete will have a minimum of 1 week which may be extended.

The athlete should present himself on 07/16 at 8:00 AM in the Resscada Stadium (reception) in Florianopolis, and ask for Alex Pereira (Supervisor) or Flavio Roberto (Technical Coordinator).

Bring the following documents:

• Bring the medical waiver attached (must be notarized or athlete will not be allowed to train)
• Bring cleats for turf
• Bring ID

- The athlete will have meals at the club during the evaluation

- No housing will be provided during evaluation

- If the athlete passes the evaluation Avai will provide housing, meals, and expenses
