National Healthcare Decisions Day is recognized each year on April 16 – the day after the date traditionally known as tax day – as a way to engage the American public in conversations that encourage people to think about their own personal wishes for their health care, particularly at the end of life, with the goal of getting people to plan ahead, document those plans, and let loved ones know their wishes.

The Conversation of Your Life (COYL) program provides free education, support, and resources to help you and those who matter most to you make decisions about the type of care you would want in a medical emergency or serious illness.
COYL hosts educational community events across the state.

1. Advance Directives: These include a **living will** and a **durable power of attorney for health care**. A living will outlines the medical treatments you would or would not want if you're unable to communicate, while a durable power of attorney for health care designates someone to make medical decisions on your behalf.
2. Health Care Proxy: This is similar to a durable power of attorney for health care, where you appoint someone to make health care decisions for you if you're unable to do so.
3. Authorization to Release Health Information: This document allows healthcare providers to share your medical information with designated individuals.
4. Personal Medical History: A record of your medical history, including past surgeries, medications, and any chronic conditions.
5. Insurance Information: Copies of your health insurance cards and any long-term care insurance policies.
