You could always put the numbered content pages in a separate document where the page numbers matched. Then place any "un-numbered" pages in reference documents. So if the table of contents is in the first 17 pages place it in a T.O.C. document. Then have a content document where page 1 is the first page, then an index document where you could look up page numbers and find them correctly in the content. I have edited the document in Acrobat to add the form fields and now I simply need to change the page to letter size. So far, I have been unable to find anywhere in Acrobat that you can do this. I think I may have to bite the bullet and just retype the twenty pages. I already tried copy and paste to a new Pages file and to Text Edit and both resulted in the text mixed in with a bunch of characters that would take a while to delete.

If you only have to add to the pdf file then the pdfpages package could help. But if you have to change anything on the pages your co-author has written then you're out of luck and your co-author will have to send you the latex file.

I have extensively tested the functionality of pdftk and exiftool. I have used exiftool both at command line and through a graphical window. These have been tested for small, medium size and very large PDF documents and found to have issues with the largest and most complex PDF documents. In my experience, the pdftk / exiftool have top functionality only for small and for simple-in-formatting PDF documents. For large and complex PDF documents (eg more than 80 pages with multiple fonts) images and/or characters may fall out from the last pages after the metadata has been edited.
