This form needs to be filled in and submitted before a file is made for your case studies. Please fill this in, scan and send it to certification@mpstherapy.com

This form should be used prior to treating patients/clients with MPS
Use this form for DATA for Standard Protocol and Battlefield Acupuncture
You need 10 of these for Battlefield and 10 for Standard protocol
Use this form for your Scar DATA
You will need 10 of these completed before you can get your test for certification
Use these instructions for your case study. Narrative formats are also welcome so long as you are illustrating from session to session basic "SOAP" information. You may also submit an article or special project in leu of a case study. Please email me your request.
