Instructors can create anonymous surveys via Google Docs or Canvas assignments with Anonymous Grading selected. These surveys can be shared with students during semester checkpoints (after the first unit/module, during the middle of the semester, at the end of the semester).

In her CISC 191 class, Prof. Tasha Frankie asks her students for feedback specific to the Zoom class sessions. She uses a Google Form (right) as a space for collecting anonymous feedback from students in the "Keep - Start - Quit" format which includes the following. Notice in the introduction how she shares with students why she's asking for this information.

I'd like to know how Zoom classes are or aren't meeting your learning needs for CISC 191. Please fill out this survey to help me understand and to be able to make informed decisions about future classes.

Notice that she provides an additional opportunity for students to add personal comment as well as the option for them to identify themselves so that she can follow-up with the student individually if necessary.
