Step 1: Be Open to Membership
Anyone can join Mochi Health, regardless of age, gender, or location (as long as they operate in your state).
Both teens and adults are welcome to access the program for weight management.
Step 2: Meet the Criteria for Medical Support
To receive personalized treatment plans (including doctor and dietitian support), there’s no strict BMI requirement—just complete an intake form and select your plan.
Step 3: Meet Prescription Eligibility (for GLP-1 Medications)
You must meet one or more of the following:
BMI ≥ 30 kg/m²
(Considered obesity; no other condition required)
BMI ≥ 27 kg/m² + 1 weight-related condition
(e.g., high blood pressure, type 2 diabetes, high cholesterol)
BMI ≥ 20 kg/m² + currently using a weight-loss prescription
(Already prescribed GLP-1 or similar from another provider)
Step 4: Teens Are Also Eligible
Teens under 18 may qualify if their BMI is above the 95th percentile for their age and sex.
They can receive both behavioral and medication-based support through Mochi.
Step 5: Complete an Online Assessment
Answer questions about your weight, health history, medications, and goals.
Based on this, a provider will determine the best treatment plan or prescription for you.
Go to Mochi Health and take the eligibility quiz to determine your initial fit for the program. This includes basic questions about your BMI, health conditions, and background.
If you're eligible, create your account and purchase a membership plan. Plans include:
1‑month ($79)
3‑months ($199)
6‑months ($399)
12‑months ($799)
Fill out detailed questions online about your medical history, medications, lifestyle, and (if applicable) insurance. Completing these before your appointment is highly recommended.
Use the portal to book your 100% virtual visit with a board-certified provider or dietitian. You can generally schedule an appointment within one week of signing up.
During your consult, your provider will review your intake information and discuss treatment options. If you qualify, a prescription plan (e.g. semaglutide or tirzepatide) may be issued. For insurance users, prior authorization may be required.
If medication is part of your plan:
Compounded semaglutide — ~$99/month
Compounded tirzepatide — ~$199/month
Once your medication arrives, begin your treatment and use the online portal to communicate with your provider, request refills, track progress, and attend follow-up visits.
