Complete the forms in as much detail as possible. in the ABOUT THE INCIDENT" section try to get a statement direct from the patient.

This form is triplicate.

• Top Copy - ours- retain and return to HQ

• Middle Copy - sent to production office

• Bottom Copy - patient copy if needed
