Drug Profile
Ubidecarenone is a naturally occurring coenzyme involved in electron transport in the mitochondria. It is claimed to be a free radical scavenger and to have antoxidant and membrane stabilising properties. It has been given orally as an adjunct in cardiovascular disorders ( Refer to ). It has also been tried in other conditions associated with coenzyme deficiency, and is promoted as a dietary supplement. Ubidecarenone has been investigated for the management of cancer, Huntington's chorea ( Refer to ), Friedreich's ataxia, and parkinsonism.

Ubiquinol , the reduced form of ubidecarenone, has also been used.

Administration in children
Inherited primary ubidecarenone deficiency results in mitochondrial disorders that cause a clinically heterogeneous group of renal, neurological, myopathic, and multisystem diseases. Treatment is largely supportive and the disorder usually progresses causing significant morbidity and premature death. Ubidecarenone has shown benefit in the treatment of some patients with inherited primary defects and early diagnosis and supplementation is important to limit symptoms and prevent organ damage.
Although unlicensed in the UK, the BNFC states that ubidecarenone may be given for the treatment of mitochondrial disorders. Suggested oral doses, given once or twice daily with food, according to age, are:
neonates: 5 mg, adjusted according to response, up to 200 mg daily
children 1 month to 17 years: 5 mg, adjusted according to response, up to 300 mg daily

Cardiovascular disease
Ubidecarenone deficiency is associated with cardiovascular disease, and supplementation has therefore been promoted as an adjunct in the prevention or treatment of various conditions including hypertension, hyperlipidaemias, and heart failure.1-4It has also been studied in the prevention of cardiac surgery complications.5 However, most studies have been small or unreliable, and evidence for a beneficial effect remains inconclusive.
