To qualify for Medical Baseline Program, a full-time resident in your home must have a qualifying medical condition and/or require use of a qualifying medical device to treat ongoing medical conditions. Only one Medical Baseline application per household is required.

Note: Qualifying medical devices include any medical device used to sustain life. Devices include equipment used for mobility, per a licensed medical practitioner. Devices are for home-use only. Devices used for therapy generally do not qualify.

Customers who do not have a permanent qualifying medical condition will complete a Self-Certification after the first year and will be required to recertify with a medical practitioner's signature for the following year.
