My MCN application journey
Step 1A: Download and accomplish the following forms
Recommendation form from Class Adviser* and
Recommendation form from Guidance Counselor*
*To be placed in a sealed envelope and signed on the flap
Additional Forms/Documents to submit as needed:
Certification of accreditation of the Homeschooling Program
(for grades 10 and 11 applicants, if available)
Photocopy of National Career Assessment Examination (NCAE) results
Step 1B: Secure the following documents
Certified True Copy of the following
Most recent current level *report card (SY 2023-2024)
*PEPT Certificate for those coming from DepEd non-accredited schools
Previously completed grade level report card (SY 2022-2023)
Original copy of the Philippine Statistics Authority (PSA) birth certificate
Clear copy of Baptismal Certificate (for Catholics)
Two (2) pieces 2x2 recent colored ID picture
Step 1C: Add additional documents for students studied abroad
Original Transcript of Records with English translation and duly authenticated by the Philippine Foreign Service Establishment located at the student’s country of origin or legal residence
Report cards should have the English translation for foreign students from a foreign school
Certification from previous school indicating promotion to the next level
Original and photocopy of updated Passport and Visa of parents and student
Photocopy of Alien Certificate of Recognition / I-Card (present original for verification)
BI Form 2014-02-005 Rev 0/CGAF Form (form provided by MCN)
Certificate of Recognition as a Filipino if with dual citizenship
Step 2: Submit the complete requirements and pay the application fee
Submit the complete accomplished forms and documents at the admissions window.
Wait for the admissions staff to review the documents and encode the application.
Once encoded by the admissions staff, proceed to the cashier window for payment of the P700 non-refundable application fee.
Step 3: Confirm admissions test schedule
Present receipt of payment to the admissions staff.
For Preschool, assessment schedule will be given.
For Grades 1 to 11, admissions test schedule will be given.
Step 4: Come on time for the admissions test or assessment schedule
Step 5: Release of decision letter
Anticipate within ten (10) working days from the date of test or assessment the decision letter to be sent via email
Step 6: Settle the reservation fee on or before the deadline indicated in the decision letter to secure the slot until enrollment.
