Premise registration is the first step you can take to protect your investment in Illinois' livestock industry. A database of locations where livestock are produced, raised and kept also aids animal health officials.

All establishments providing body art services such as tattooing, piercing, branding or scarification are required to be registered with the Department. Upon receipt of a completed registration application, an inspection is conducted to monitor compliance with the program. Renewal inspections are conducted periodically. Certified Local Health Departments may contract with the Department to conduct the inspections in their jurisdiction.

There is a MINIMUM of 2 sinks required for every body art shop - 1 in the restroom and 1 in the work area that is used for nothing else but the artist for handwashing. See sections 797.100 and 797.1100 g and h for further details. Portable sinks are NOT ACCEPTABLE.

Only Operators are allowed to register for body art registration certificates. Pursuant to Section 15 of the Tattoo and Body Piercing Establishment Registration Act, only Operators are legally permitted to apply for body art registration certificates. The past practice of accepting applications from persons other than Operators, including landlords who rent premises upon which such operations are conducted will no longer be followed. The landlord of a location cannot register the location they are renting out to an Operator but the Operator itself must obtain and maintain their own registration and be responsible for the functions in their own business at the address.

This state registration takes care of the IDPH requirements. PLEASE remember to check for local business and zoning requirements before attempting to gain the state registration. Your city and county government may both have additional requirements to make the location compliant.

 Fully equipped mobile units applying for the regular year registration shall pay the $500 fee, (plus $50 more per additional station).

 If it is not fully equipped and does not have a bathroom in the unit it can only apply as a temporary unit registration which is $250 per 14 consecutive days.

Any domestic or foreign limited liability partnership (LLP), limited liability limited partnership (LLLP), and any domestic or foreign restricted professional company (PLLC) in existence on December 31 of any year is required to file a Certificate of Annual Registration. This form and the corresponding annual registration fee must be filed on or before April 15 of each year following the calendar year for which it is being filed.

If an owner, partner or officer of the company that owns or operates the facility has a felony or misdemeanor conviction or forfeit since the last facility certificate renewal, complete the Facility Renewal Application (VS-2) and a Statement of Ownership and/or Permission to Use Place of Business (pdf) (VS-19) send it to us by mail.

Once the application is complete, an enforcement officer will inspect the facility or mobile repair vehicle to ensure that it complies with the requirements of the Miami-Dade County Code. Once the motor vehicle repair business passes inspection, a motor vehicle repair registration will be issued authorizing the business to perform specified repairs.

Any person(s) or corporation that operates a motor vehicle repair facility in Miami-Dade County whose registration is expiring must renew at least 30 days prior to the expiration date of the current registration.

Individuals performing motor vehicle repairs on behalf of a registered motor vehicle repair business must also register with The Department of Regulatory and Economic Resources as either a certified or apprentice mechanic or technician. Mechanics must take a test to demonstrate competency and must take 16 hours of continuing education every year. Apprentices must be employed by a motor vehicle repair facility which has obtained a registration and shall be required to work under the direct supervision of a certified motor vehicle mechanic/technician.

Changes in any material aspect of the motor vehicle repair facility shall be provided to Miami-Dade County in writing along with proof of the change and the required fee, if replacement of the registration is required, within 30 days of the change.

Businesses that sell tangible personal property or taxable services in New York State need a Certificate of Authority. The certificate allows a business to collect sales tax on taxable sales. The certificate comes from the New York State Department of Taxation and Finance (DTF).

Agents and brokers who are new to the Marketplace this year, or who did not complete plan year 2023 registration and training, are required to take the full Individual Marketplace training for plan year 2024. New agents and brokers can take Individual Marketplace training through the MLMS or the HHS-approved vendor(s).

Agents and brokers who completed plan year 2023 Individual Marketplace registration and training are eligible to take a shorter training for plan year 2024 as well as optional review modules. Returning agents and brokers can take training through the MLMS or the HHS-approved vendor(s) once available. Returning agents and brokers are eligible to take a condensed training to complete the Individual Marketplace training requirement. The required portion of Marketplace Training for Returning for Returning Agents and Brokers takes approximately on hour to complete. The curriculum consists of one required training module, one required exam, and signing the Agent Broker General Agreement. Returning agents and brokers can take training through the MLMS or the HHS-approved vendor(s).

The Agent and Broker FFM Registration Completion List (RCL) contains the National Producer Numbers (NPNs) for agents and brokers who have completed Marketplace registration for the current year. CMS posts this list as frequently as daily. All NPNs are self-reported by the agent or broker during Marketplace registration, and should be validated against state and/or other The National Association of Insurance Commissioners (NAIC) records to confirm state licensure. CMS is updating the agent/broker licensure validation methodology in some states. Agents and brokers who do not have an approved health-related Line of Authority (LOA), as determined by their resident state, do not have access to Marketplace systems and are not allowed to assist consumers with Marketplace enrollment. Agents and brokers can check their resident state licensure requirements to ensure they have an approved health-related LOA. State licensure questions should be directed to the resident State Department of Insurance authority found at the National Insurance Producer Registry (NIPR) at -center/add-a-line-of-authority.

The Agent and Broker FFM Registration Termination List (RTL) contains the NPNs for agents and brokers whose FFM agreements and registration for the 2018 plan year have been terminated by CMS. Termination is described in regulation at 45 CFR 155.220(f)-(g).

The proprietors who run a business from home without having any physical store or premises are also required to obtain this Certificate. The proprietors of e-commerce business or online business, or online stores and establishment must register under this Act and obtain the Certificate.
