Integrity Colorado PASA Nurses, LLC—or ICPN—is an approved Intellectual/Developmental Disability (I/DD) Program Approved Service Agency (PASA) or DD Waiver approved agency, providing service and support to adults, 18 and older, with employed family members/friends approved by Health First Colorado Home and Community-Based Services (Medicaid) Waivers.
Gayle and Tammy have 57 years of combined Nursing Experience and 17 years of I/DD Experience. Their goal is to support and guide the client and family through the transition from the Children's Extensive Supports (CES) waiver to adulthood and all that the DD waiver offers, working with the individuals case managers to maintain in home and community-based services, while engaging the client in planning, and participating in activities that build confidence and independence as well as receiving the care they require from their loved ones and community.
The DD Medicaid Waiver provides assistance up to 24 hours/day, 7 days/week according to an individuals priority of needs. This program includes a range of services, such as residential (in the home), day programs, behavioral, transportation, and employment assistance. Enrollments are limited by the state and there is a waiting list. Those on the CES are able to transition to the adult I/DD Waiver at the age of 18, if qualified. The process can begin up to a year prior to turning 18. Many goals are met with the help of our agency, enabling a smooth transition.
