Step 1: Reserve your plate

Mail it via USPS with a $25 deposit (check) to the Illinois Secretary of State. You'll find the address in the upper left corner of the form.
Your $25 goes into a special fund reserved for the youth tennis license plate decal (whether or not you ultimately transfer your plates or not).
Step 2: Wait for the 2,000-driver goal
Once 2,000 people have reserved a plate, the state will begin producing the plate decal.
Step 3: Complete your plate order
When production starts, you’ll receive a letter from the Secretary of State with a universal plate application.
At that time, you’ll complete the form and pay the remaining $15 cost for the decal (the original $25 + the additional $15 = $40 for the total annual cost of the youth tennis decal).
You’ll then receive your new universal plate with the youth tennis license plate decal.

The decal can only be used on a universal plate. You can apply for a universal plate before or at the time of your license expiration; the final cost depends on the remaining time left on your current registration.
You don’t have to wait until your current plates expire; you can apply anytime.
Once ready, the Secretary of State will mail you your new plate and decal.
