Rx:

Inj NPH 4-0-4 units s/c

Inj Regular insulin 4-4-2 units s/c

Planned correctional insulin 1 unit per 100 mg/dl; before breakfast, lunch and dinner

You can see that the regimen has been left unaltered. The days 8 PM BS is lower than that of the day before. BS is on a declining trend. Further change needs to be decided on next days values. If the trend is lower insulin will have to be reduced or the patient needs to be informed about the chance of hypoglycemia and monitored.
