Rx:

Inj NPH 12-0-10 units s/c

Inj Regular insulin 8-8-6 units s/c

Planned correctional insulin 1 unit per 40 mg/dl; before breakfast, lunch and dinner

The regimen has been updated based on the total requirement of 43 units the previous day. Basal and Prandial insulin will each be given at 50% of that dose (22 units). Further correctional insulin as required. (1 unit per 40 mg/dl higher value than target [1800/43 = 41])
