Initial Rx

The target BG can be between 140 and 180 mg/dl.

Metformin needs to be stopped as the patient has an infection and insulin needs to be given.

This patients RBS was 210 mg/dl and his prior treatment has been an optimal dose of Metformin. His insulin requirements are expected to be low.

Sliding scale insulin can be chosen here initially for BS more than 200.

E.g., 2 units s/c insulin for CBG of 250 mg/dl, one unit extra for each 50 mg/dl higher value.

Monitoring and treatment being once in six hours.
