Initial Rx
1. Glimepiride needs to be stopped and substituted by insulin. (1)
2. Basal bolus regimen with a total dose of 0.3/Kg/day units of insulin (lower end of starting dose) can be started. (2)
Better to avoid correctional insulin on the first day.
