To register your child for our Summer and/or Fall Programs: SafeCare Registration Form
For Toddler, Preschool or Pre-K students (in addition to the Registration Form): ODJFS Medical Form*
*This form must be completed by your child's doctor BEFORE his/her first day at SafeCare.
For SUMMER PROGRAMS: ODJFS Enrollment Form (Summer 2025)**
For FALL PROGRAMS: ODJFS Enrollment Form (School Year 25/26)**
**If registering for summer AND fall, you only need to complete the ODJFS Enrollment Form for Summer (as well as the Registration Form).

Optional Forms

To enroll in autopay, or to update the payment method used for autopay: Payment Authorization Form
To add or remove persons from the Authorized Pickup List for your child(ren): Authorized Pickup Changes
If a Child Medical/Physical Care Plan for Child Care is required: ODJFS 01236
If a Request for Administration of Medicine is required: ODJFS 01217
