MyChart offers eCheck-in for most appointments. Before your appointment, you may receive an email and text message asking you to complete the eCheck-in process for your upcoming appointment. During this process, you will be asked to complete questionnaires, sign documents, review and update your medication list, upload your ID and insurance card before your appointment time.
If you are unable to complete eCheck-in ahead of time, you may be asked to complete questionnaires, sign documents, update your medication list and fill out additional paperwork when you arrive for your appointment. Completing the eCheck-in process saves you time in the office and expedites your care experience.
4. Select "Non-Medical Question". Please note that documents uploaded in this fashion will NOT be reviewed by a doctor or APP unless a visit occurs, and are only for purposes of sending school forms to be completed or to be included in the patient's chart. For clinical questions or concerns, please select Medical Question (Non-Urgent).
Sending documents & images (like Patient Registration or insurance card images) is an easy & secure process: you will compose a New Medical Question and attach the files. These files go directly into your chart and are available to staff and providers as needed.
Advance care planning involves making decisions about the type of health care you would want if you became too sick or were unable to tell the doctor yourself. These preferences are often put into an advance directive, a legal document that goes into effect only if you are incapacitated and unable to speak for yourself. It helps others know what type of medical care you want. You can add your advance care planning documents directly into your MyChart account so they can be found when needed.

Lucy is a feature of MyChartCentral that allows you to view, enter, and manage your personal health information. With Lucy, you can enter information about your health that you can send to your healthcare providers. You can also request copies of your medical record and review them. You can even upload medical documents.
The MyChart by Epic login website is mychart.hhsc.ca/MyChartEpicPRD/ and the app looks like a red folder with a heart on it. Sign up for your account with an activation code or link from your health-care provider.
If you require proxy access to a MyChart account of someone who cannot consent, reach out to Health Records at epicmychart@hhsc.ca to discuss what options you have, and what documentation you may need to provide to support the proxy provisioning.

The information displayed in MyChart does not represent the complete medical record , e.g., diagnostic images, scanned documents or information maintained in other systems. Posting of information into MyChart is at the discretion of your individual health care providers(s) and our organization. Contact our Health Information Management Department at 760-773-1211 for official copies of medical records not viewable in MyChart or in regard to discrepancies or potential errors with the medical information listed in your record.
