Hormonal Contraceptives Market: A Steady Growth Trajectory Through 2027
The global hormonal contraceptives market is on a consistent upward path, driven by rising awareness of family planning, expanding healthcare access, and continuous product innovation across the world. Hormonal contraceptives, which work by regulating a woman’s reproductive hormones using synthetic estrogen and progesterone, remain among the most widely adopted birth control methods globally, spanning pills, injectables, vaginal rings, intrauterine devices, and implants.
Market Size and Growth Outlook
According to Fortune Business Insight, the global hormonal contraceptives market was valued at USD 16,866.1 million in 2020 and is projected to reach USD 24,330.1 million by 2027, expanding at a compound annual growth rate (CAGR) of 5.4% during the 2020–2027 forecast period. According to Fortune Business Insight, the market had stood at USD 17,620.9 million in 2019, before experiencing a decline of 4.3% in 2020 compared to the average year-on-year growth recorded between 2016 and 2018 — a dip largely attributed to pandemic-related disruptions in healthcare access.
North America has historically led the global landscape. According to Fortune Business Insight, the region accounted for a 39.03% share of the global market in 2019, with its value rising from USD 6,512 million in 2018 to USD 6,877.6 million in 2019. This dominance stems from the strong presence of major pharmaceutical companies, rising incidences of unintended pregnancies among teenagers, and growing awareness of various contraceptive options across the United States.
The Pandemic’s Impact on Accessibility
The COVID-19 pandemic significantly disrupted the supply and accessibility of contraceptive products worldwide. According to Fortune Business Insight, roughly two-thirds of the 103 countries surveyed between mid-2020 reported disruptions to family planning and contraception services. This loss of access carried serious downstream consequences: estimates cited in the report suggest that a 10% annual decline in contraceptive use due to stock-outs, clinic closures, or unavailable providers could leave an additional 48.5 million women with unmet contraceptive needs, potentially resulting in over 15 million additional unintended pregnancies globally. Such disruptions temporarily slowed market momentum, even as long-term demand fundamentals stayed intact.
Key Drivers Fueling Market Expansion
A major growth driver remains the substantial unmet need for contraception, particularly in developing regions. According to Fortune Business Insight, around 24.2% of women of reproductive age in Africa currently have an unmet need for modern contraception, compared to 10.2% in Asia and 10.7% in Latin America and the Caribbean. This persistent supply-demand gap presents considerable opportunity for market players to expand distribution and awareness campaigns in underserved areas.
Government-led initiatives promoting family planning and reproductive health, combined with the steady introduction of easier-to-use hormonal contraceptive products, are also propelling adoption. Vaginal rings and subdermal implants, in particular, have gained strong traction in developed markets due to their convenience and long-lasting efficacy. According to Fortune Business Insight, one such implant product is regarded as more than 99% effective at preventing pregnancy and can remain functional for up to three years, making it an increasingly popular choice among women seeking low-maintenance contraception.
Restraints: Side Effects and Health Risks
Despite favorable growth prospects, the market faces constraints tied to the side effects associated with hormonal contraceptive use. According to Fortune Business Insight, common adverse effects include headaches, nausea, breast tenderness, and vaginal infections, along with irregular spotting and mood changes. Of greater concern is the elevated risk of blood clot formation, particularly among women over 40, smokers, and those with higher vascular disease risk. These health considerations continue to influence consumer choice and prescribing patterns, somewhat tempering the pace of market expansion.
Segmental Insights
By product type, the pills segment continues to command the largest share of the market, owing to improved accessibility, affordability, and regulatory approvals for combination oral contraceptives. According to Fortune Business Insight, the pills segment is projected to generate USD 16.53 billion in revenue by 2025. Meanwhile, implants are expected to post the fastest growth rate among all product categories during the forecast period, reflecting rising preference for long-acting, reversible contraception.
On the distribution front, hospital pharmacies held the largest share of the market in 2019, supported by rising demand for irreversible contraception methods and the expansion of multispecialty hospital networks, especially across emerging economies. According to Fortune Business Insight, the hospital pharmacy segment is anticipated to command a 37.2% share of the market by 2025. Retail pharmacies and online distribution channels are also expected to see considerable growth, fueled by expanding retail infrastructure and the broader shift toward e-commerce-based healthcare purchasing.
Regional Dynamics
Beyond North America’s leadership position, Europe stands as the second-largest regional market. According to Fortune Business Insight, Europe is projected to grow at a CAGR of 5.6% during the forecast period, with Germany emerging as a particularly strong performer due to expanding reimbursement coverage for contraceptive products.
Asia Pacific, meanwhile, is poised to register the fastest regional growth rate, driven by expanding pharmaceutical infrastructure in countries such as India and Japan, along with heightened government focus on population control measures and reproductive health awareness. According to Fortune Business Insight, China alone is expected to post a robust CAGR of 5.90% during the forecast period.
Competitive Landscape
The hormonal contraceptives market remains fairly consolidated, with a handful of major pharmaceutical companies controlling a significant portion of global revenue. According to Fortune Business Insight, key players include Merck & Co., Inc., Allergan, Teva Pharmaceutical Industries, Pfizer, Inc., Mylan N.V., Novartis AG, and Janssen Pharmaceuticals. Merck & Co. holds a particularly strong position due to its widely used vaginal ring and implant product lines, while Bayer AG maintains a notable presence through its range of contraceptive pills and intrauterine devices.
Recent industry developments underscore the sector’s continued innovation. According to Fortune Business Insight, regulatory approvals for new oral contraceptives — including products derived from plant-based estrogen sources — were granted in both Europe and the United States in 2021, reflecting ongoing R&D investment aimed at improving safety profiles and reducing side effects.
Conclusion
The hormonal contraceptives market is set for sustained growth through 2027, underpinned by rising global awareness, expanding healthcare infrastructure, and a steady pipeline of innovative products. While pandemic-related disruptions and side-effect concerns pose near-term challenges, the substantial unmet need for contraception in developing regions, combined with strong institutional and governmental support for family planning, positions this market for continued expansion in the years ahead.
