Duties and Responsibilities

• Patient Assessment and Care Planning: Conducting initial assessments, developing individualized care plans, and monitoring patient progress.

• Medication Administration: Administering oral, injectable, and topical medications as prescribed.

• Wound Care: Cleaning, dressing, and monitoring wound healing.

• Personal Care: Assisting with bathing, dressing, grooming, and toileting.

• Documentation: Accurately and timely documenting patient care activities, observations, and interventions.
