Notice of Intent to Establish a Home Education Program
This form must be filled out and submitted to the home-school department of the county in which you reside within 30 days of the start of a home education program.

Notice of Intent to terminate a Home Education Program
The parent shall file a written notice of termination upon completion of the home education program with the district school superintendent, along with the annual evaluation required, within 30 days of termination.

The annual Home-School Evaluation is due one year after the Intent to Establish a Home-Education has been filed.
