Harrison Mariki is a health supply chain management expert with extensive experience building the capacity of people and systems supporting health supply chains in Tanzania and has used this experience to provide technical assistance in Kenya, Myanmar and Indonesia. He has extensive field experience strengthening data collection, integrity and use at service delivery points for logistics decisions through implementation of innovative solutions like the electronic logistics management information system (eLMIS) and the Vaccines Information Management System (VIMS). He has also supported implementation and integration of electronic immunization systems (EIS) in Tanzania. His current work focuses on thought leadership and driving innovation towards supply chain excellence, managing, mentoring and building supply chain talent within the region.

Availability of health commodities is fundamental to health systems and success of public health programs in low- and middle-income countries (LMICs) across the board from Immunization to HIV/AIDS, preventing child and maternal deaths, malaria, reproductive health, dealing with COVID-19 and all areas of care that ensure public health.
Availability of health commodities is fundamental to health systems and success of public health programs in low- and middle-income countries (LMICs) across the board from Immunization to HIV/AIDS, preventing child and maternal deaths, malaria, reproductive health, dealing with COVID-19 and all areas of care that ensure public health.
In low- and middle-income countries (LMICs) the availability of essential medicines in public health facilities ranges from 17.9% to 87.1% (WHO, 2014). This poses great risk on success of these programs and the potential outcomes in terms of lives that can be saved. I have always taken interest in learning best practices in business and management fields with the hope of finding best practices that I can apply to improve health supply chains.

What does an Ideal Supply Chain in a low or middle-income country look like? Defining this future is a step closer to achieving consistent availability of healthcare products and services. To answer this question we will need to put the client at the center and design supply chains around them, what has recently been coined as patient-centric supply chains.

A supply chain vision that compromises any aspect of serving the client better will not take us any closer to achieving objectives of healthcare systems, so we need to be bold in deciding this vision. In his book, As a Man Thinketh, James Allen says “To Desire is to Obtain to Aspire is to Achieve” This call for a bigger vision is very applicable to health supply chains as to any other field.

Management by exception (MBE) is a style of business management that focuses on identifying and handling cases that deviate from the norm, recommended as best practice by the project management method. To succeed, this principle requires data should be used in measuring progress towards achievement of a supply chain performance goal (e.g. what percent of service delivery points have all the needed commodities consistently available?)

This is well supported by continuous quality improvement principles like the PDCA (Plan-Do-Check-Act Cycle) and has widely been applied in private sector supply chain improvement strategies.

When presented with a situation where only 20% of the service delivery points are able to meet 80% of a supply chain performance goal (e.g. have all the needed commodities consistently available), supply chain managers tend to focus on improving the 80% by doing problem solving and action planning. In such a situation where the majority of the service delivery points are under performing, Focusing on the 80% poor low performing service delivery points overwhelms supply chain managers given the limited resources. The Pareto Principle applied well can lead to great results in this context. It is a tried and tested management technique that aids you in decision making by using the 80/20 principle.
