For minors, the permission of the guardian/parent is necessary! Download it here. The tattoo artist will not start without a signed guardian/parental consent! A new consent is required for each tattoo! Attached with the above-mentioned parental consent you will receive a declaration of the artist to the guardian/parent that the artist is informed and has checked thoroughly all the IDs or passports about the age of the child. These documents are given to the guardian in the original signed form, but they are photographed/scanned and kept as digital copies by the artist. The artist bears the responsibility of preserving them in digital form for 20 years from the creation of each tattoo. After 20 years, the artist may delete the documents without informing the signatories. You can print and sign it by hand or if you prefer it to be more official you can use an online digital signature platform of your country.
