Travel Notification Forms

In the event that you will be transporting your athlete instead of them traveling with the team on the bus to scheduled competitions, it is crucial that the forms below be utilized. 

Per GFPS athletics, below is the link to the form that parents need to fill out prior to driving their son/daughter to a game.  In order for us to accurately request busing, we would like all parent requests to be submitted 48 hours prior to departure.  Please sure this link and the timeline with your parent contact lists.
