Objective:  Conclusions regarding lithium's antisuicidal effect for bipolar disorder have been limited due to nonrepresentative subjects and potential confounding factors, including varying severity of illness. Findings regarding the effect of valproate, the most common alternative to lithium, are inconsistent for suicidal behavior. This study investigated the associations of these two drugs with the risk of suicide-related events, and possible differences between drugs, by using within-individual designs in a register-based longitudinal cohort.

Method:  Through linkage of multiple Swedish national registers, 51,535 individuals with bipolar disorder were followed from 2005 to 2013 for treatment with lithium and valproate. Stratified Cox regression was used to estimate the hazard ratios of suicide-related events during treated periods compared with untreated periods. For significant associations between medication and suicide-related events, the population attributable fraction was estimated to assess the public health impact for patients with bipolar disorder.

Conclusions:  The results suggest that lithium should be considered for patients with bipolar disorder with suspected suicidal intentions, although risk for suicide is only one of the considerations when providing clinical care.

Today I am focusing on Evanescences song Lithium. Lithium is a drug sometimes associated with bipolar disorder since it has been fairly successful in helping stabilize manic and depressive episodes. Lithium was first used to treat mania in 1970. Kay Jamison wrote about the benefits of the drug in An Unquiet Mind stating

I think what is particularly interesting and controversial about Amy Lees song Lithium is that she claims to have no experience with taking lithium, and still uses it as a metaphor for the entire song. Not only does she not have experience with the drug but also she uses it in a negative contextequating it to numbing ones selfmuch as Kay Jamisons sister accused her of doing while taking lithium.

There are many metaphors that Amy Lee could have chosen to get at the idea of numbness. I would be interested to ask her why she chose lithium. I would guess that she may have chosen it due to the relationship between creativity and bipolar disorder and her songs exploration of her own creativity being linked to sorrow. Although Amy Lees song equates lithium to happiness and a lack of creativity, interestingly one of the abilities Kay Jamison still had when taking the extremely high doses of lithium was her ability to write poetry.

However, I kept changing my mind and changing it back. Who would be interested in reading about how any of my songs came about? And why would I want to write about such personal, usually painful, things?

The first backstory, included below, is for a song I wrote shortly after the ending of a relationship I had with a man afflicted with bipolar disorder. The relationship was wonderful and miserable, surrounded by music and art, and woven with love and madness. As painful as it all was, I have to admit that there were beautiful moments and lessons learned that helped form the fabric of who I am and why I am the way I am today.

Musical obsessions are generally considered a symptom of obsessive-compulsive disorder (OCD). But between 15 and 20 percent of people with bipolar also meet the criteria for OCD, according to the Journal of Affective Disorders.

Although bipolar electrograms (Bi-egms) are commonly used for catheter mapping and ablation of cardiac arrhythmias, the accuracy and reproducibility of Bi-egms have not been evaluated. We aimed to clarify the influence of the catheter orientation (CO), catheter contact angle (CA), local conduction velocity (CV), scar size, and catheter type on the Bi-egm morphology using an in silico 3-dimensional realistic model of atrial fibrillation. We constructed a 3-dimensional, realistic, in silico left atrial model with activation wave propagation including bipolar catheter models. Bi-egms were obtained by computing the extracellular potentials from the distal and proximal electrodes. The amplitude and width were measured on virtual Bi-egms obtained under different conditions created by changing the CO according to the wave direction, catheter-atrial wall CA, local CV, size of the non-conductive area, and catheter type. Cardiac arrhythmias are rhythm disorders of the heart leading to abnormal heart function. For the diagnosis and treatment of the arrhythmias, clinicians insert catheters into the heart and examine the electrical signal propagation in the heart. Among different type of catheters, bipolar catheters have two electrodes at the tip of the catheter with the signal being the difference between the two electrodes, which provides sharper signal than unipolar catheter. However, bipolar electrogram is dependent on many factors including catheter design and orientation, and consequently, knowledge of the determinants of the bipolar electrogram is needed for proper interpretation of the signal. In this study, we examined the effects of many factors on bipolar electrogram using computer simulation. Computer simulation is very useful in this type of study because, in clinical settings, it is not feasible to control each factor precisely. We quantitatively demonstrated the effects of catheter design and orientation, and cardiac wave propagation speed on bipolar electrogram.

A representative Bi-egm strip obtained for the virtual Thermocool catheter in the 3D atrial model with induced AF is provided in Fig 2A. Compared to the sinus rhythm conditions, the AF conditions produced a lower bipolar amplitude (1.770.53 vs. 1.470.39 mV, p = 0.013; Fig 2B). That seemed to be due to the random direction of the activation wave with respect to the catheter orientation. On the other hand, the bipolar amplitude did not change significantly upon reducing the APD90 from 223 to 180 ms (Fig 2B).
