I was gonna update my phone to 5.1.1 however it told me the upgrade have failed on the computer but my phone remind the itunes icon and usb cable and its not coming off.... i tryed everything but nothing work...please help me

I have had this exact same issue for the last year. I have been given the same generic "it's a hardware issue" for the last year. Apple replaced my phone a year ago because of this "hardware" issue however I still have the same problem with this new phone as well. I have found a way to rectify the problem. If I decide to update my phone I will do so and then I usually receive some sort of error message from itunes saying it couldn't update the software and then I get a screen with the itunes logo and a usb cable. When this happens I then have to connect it to a 2nd computer that has itunes on it. The phone will then continue with the update. When the updating is done on the 2nd computer I then return the phone to the 1st computer where it will now connect and allow me to restore the phone with my backup information. How this could possibly be a hardware issue is beyond me. It clearly is a software issue that is not being addressed. Don't get me wrong I LOVE apple products and wish I had more but this issue I have had for the past year with updating my phone is a pain in the but hassle and I pray that the "it's a hardware issue" line would stop cause it's not.

Also, every time I make an edit to a playlist on my phone, and then sync it to iTunes right after, the playlist shows up in iTunes as being empty.

i followed all the steps with the USB cable, i have 3rd generation apple tv. and it's still blinking the light and showing the same usb cable/itunes logo screen. Most frustrating. I don't know what to do. First maverick updated caused a crash in my laptop, now the new update on apple tv has caused it to stop working! It's a new feature in the Post-Jobs era of apple. It seems they seem to have lost some of their common sense... You need to buy a micro USB - and yes you have to unplug from the hdmi too as you probably can't fit them both at once. Then plug in and wait for itunes to reboot your aTV.

Turned out the only thing that I could do was to log out of my account and restart my Apple TV box. The picture is from what it shows is connected as a source to my Apple TV app on my iPhone. All that are not on are not on my phone or my Apple TV box.

Yeah the frustration with not having your movie libraries work is vasty old. My iPhone 7 Plus would time out every time I tried to load my library in the Apple TV app. Still to this day my Apple TV 4K still tells me when I purchase movies they'll show up here. My Mac Mini M1, which is my first apple computer on the Apple TV app it list's one movie I purchase back in February of 2022. My current iPhone 12 Max did have about 12 movies in my Library which had the latest one I purchased some two years before I had upgraded to my current phone. With all the help from support and the logging out and rebooting of all my devices my iPhone now only shows 4 movies in my library and latest purchases which are now some 5 years or so old. Only place I can see my movies and purchase history is on iTunes on my old windows computer that I fire up each year to do my taxes on. The Movies iTunes app does show my library and there is over 5k movies on there but it doesn't have the same layout and separation of the movies like the Apple TV app library does. Maybe someday they'll figure that all out.
